Oversight means acting on what you know, when you know it, to protect people who cannot protect themselves within the project governance system. A Hold Point breach is a Major NCR. It is never acceptable to allow work to proceed past a Hold Point without the designated inspector's sign-off — regardless of programme pressure, commercial pressure, or instructions from contractors.
